Age well - Heart Failure Rehabilitation Services

Healthwatch Herefordshire carried out this project in partnership with Wye Valley Trust. We gathered views and experiences of Herefordshire people that have experienced Heart Failure rehabilitation. We looked at ongoing self-management and engagements in Heart Failure care.
  • 8 people shared their experience of living with heart failure and Wye valley trust rehabilitation services.
  • Responses were from age categories 25-49 to 80-84. 
  • Most participants were male.
