Home from Hospital Report

We brought together a range of community organisations interested in developing a collaborative volunteer model to support discharge with the purpose of getting people on a straightforward discharge pathway out of hospital as quickly as possible, by unblocking some of the things they needed in place at home to be able to leave hospital.
 

The funding of £42,000 came from the Better Care Fund, was matched in part from Age UK H&W to fund a pilot for 6 months. Age UK H&W was the lead partner, who worked with their own volunteers and volunteers from HVOSS and Ross Community Development Trust. WVT worked with Age UK H&W to make appropriate linkages to the hospital and associated teams responsible for discharge while the council commissioning team held the contract.
