Herefordshire Diabetes Project
Alongside our primary care networks, we undertook a piece of work to research what effective management of Type 2 diabetes looks like.

- The project took place over a period of 12 months.
- We interviewed 75 people over the phone and via an online survey.
- We looked at the reasons as to why some people with type 2 diabetes are able to keep control of their condition and reduce the risk whilst other are less successful.
The report enabled us to understand the needs of those living with long terms conditions and how primary care servces can support these patients more successfully.
